Entering a Compound in BestRx

Entering a compound in BestRx follows a process similar to manually adding a drug, with a few additional steps to identify and define the compound and its ingredients. Once created, the compound can be reused for future prescriptions.

Drug File Details

From the RX Processing screen, ensure the Drug field is empty or that you are in New Rx mode. Press F4 (Drug File) and select Add New Drug to begin creating the compound.

The required fields are the same as when adding any other drug. The following fields are specific to compound creation and must be completed carefully:

Drug Description
Enter a clear and descriptive name for the compound. A detailed description makes it easier to locate and reuse the compound in the future.
NDC / Product ID
Enter any numeric value as a placeholder. This number is not transmitted with the claim; only the NDCs of the individual ingredients are sent. Do not use an NDC that matches an existing drug in the system, as this can cause pricing confusion during updates. Compounds use custom NDC #s typically (99999999999)
Package Size
Enter the base size for the compound being created. BestRx will automatically adjust quantities for future prescriptions, but this base record should accurately reflect the compound being prepared.
Unit of Measure
Select the appropriate unit of measure for the compound.
Drug AWP / Direct Price / Fed MAC / Special Dispense
Enter 1.00 in the Drug AWP field and tab to the next field. BestRx will automatically populate the remaining pricing fields with the same value. These fields will be updated later.
Compound Drug
Set this field to Yes to designate the drug as a compound.

Once these fields are completed, select the Compound Details tab.

Compound Details

The Compound Details tab lists all ingredients included in the compound and defines key attributes of the preparation.

Compound Type
Select the appropriate compound type.
Level of Effort
Specify the preparation complexity level. In most cases, insurers do not reimburse above Level 3. The pharmacy is responsible for determining the correct level in coordination with the insurance provider.
- 00 – Not Specified
- 11 – Level 1 (Lowest; 1–4 minutes)
- 12 – Level 2 (Low complexity; 5–14 minutes)
- 13 – Level 3 (Moderate complexity; 15–29 minutes)
- 14 – Level 4 (High complexity; 30–59 minutes)
- 15 – Level 5 (Highest; 60+ minutes)
- 16-22 (Higher levels of effort)
Route of Administration
Select how the compound will be administered.
Dosage Form
Select the appropriate dosage form.
Dispensing Unit
Select the dispensing unit. This should match the unit defined on the main drug record.

Adding Ingredients

Ingredients are added one at a time:

Search for and select the ingredient in the Drug Name / Product ID field. (Search the Master Drug File to add new drugs if you cannot locate the drug).
Enter the quantity for the ingredient.
Click Add Ingredient.

Repeat this process until all ingredients have been added. The Basis of Cost and Modifiers fields should not be used unless specifically instructed by the insurance company.

Pricing Details

After all ingredients are entered, BestRx automatically calculates the total ingredient AWP and total acquisition (ACQ) cost. As a general rule, do not rely on the quantity total when reviewing a compound, as it rarely reflects the actual finished size and may cause confusion.

Use the calculated AWP and ACQ totals to replace the placeholder 1.00 values previously entered in the pricing fields. Once all pricing fields are populated correctly, click Save & Exit.

Finalizing the Compound in RX Processing

When returned to the RX Processing screen, the compound ingredient details may not automatically populate for newly created compounds.

To correct this:

Open the Compound Details tab within RX Processing.
Click Get Details from Drug File to copy the ingredient information.

Click Back to RX Processing and continue processing the prescription as normal.

Completion

You have successfully entered your first compound into BestRx. These same steps can be followed to create and maintain additional compound records as needed.
