To access the eCare Participant File, click on eCare Plans -> B. eCare Participant File.

eCare Participant File

The Participant File allows the users to View, Add, Edit, or Delete eCare Plan participants. To Add a patient simply click "Add New Participant"; to Edit a record, click it from the list of patients as shown above.

  • Participant Type : Indicate the participant type from the drop down list.

  • Last Name : Enter the participant's Last Name

  • Suffix : Enter the participant's suffix (if applicable)

  • Prefix : Enter the participant's prefix (if applicable)

  • First Name : Enter the participant's first name

  • Middle Name : Enter the participant's middle name (if applicable)

  • Organization : Enter the participant's organization (if applicable)

  • Address 1 : Enter the participant's address

  • Address 2 : Enter the participant's second address line (if applicable)

  • City : Enter the participant's City

  • State : Enter the participant's State

  • Zip : Enter the participant's Zip code

  • Phone : Enter the participant's phone number

  • Fax : Enter the participants fax number (if applicable)

When all required fields have been filled out, you will be able to Save the participant to resume working with the list, or Save & Exit to get back to the previous screen. Alternatively if Editing a participant, you can Delete the record to remove it from the system.
